Form for 2012 Results 2013 Plans
NOTE: Content from key fields in the form will need to be copied from last year's 2012 ACTION document into this one, including each "Strategy" and reporting dates across the top row of each sheet. Remember the last 3 columns in the 2012 ACTION document (F, G, & H) represent the 2012 plan; carry each of those 3 columns into the green columns (B, C, & D) in the 2013 ACTION document. Report the results from your 2012 assessment in column E, closing out the 2012 report. Columns F, G, & H of the 2013 ACTION document represent your 2013 plan.
